Yes. At the very least this points to a possible ear infection. Left untreated ear infections can do brain damage and cause death in rabbits.

There are also some parasites and viruses that cause head tilt if your rabbit is an outdoor rabbit. Please take him to the vet.

Edit: If it's an ear infection.. No clue how those start. The same as they do in any other animal really.. If he's a lop rabbit then he's more prone to it. If it's a parasite or virus then he could get it just from being in an area with a contaminated animal or from contact with a contaminated animal depending on what it is.

How old is your rabbit?

There are a few things it could be:

1.) An ear infection. Ear infections often follow a respiratory problem or cold. So if your rabbit has been sneezing lately or the insides of it's front feet look wet or matted from wiping snot on them, then this could be the case. Feel the base of the ear and see if it is warmer than usual. If so your rabbit most likely has an ear infection. Winter is coming on so that could be a good possibility.

2.) Wry neck. Wry neck normally happens moreso with young rabbits. It is due to the rabbit not having good enough ventilation and a build up of ammonia fumes.

3.) Ear mites. Check in the ears and see if you see anything that looks like crusty scabs down inside the ear. If so, you rabbit probably has ear mites. Rabbits can sometimes get that through their bedding. Lops typically are more susceptible because their ears hang closer to the bedding.

My best guess without seeing would be an ear infection. A vet can help cure that with an appropriate antibiotic.

It could be a condition called "Wry neck or Torticollis"
Commonly called Otitis media, which is an inflammation of the middle ear. Caused by a bacteria due to Pasteurella.
Rabbit head tilts and they loose balance easily. They still eat and drink normally. The prognosis for this disease is guarded. You should see a vet immediately and ask about the condition Wry neck.
