People taking care of themselves has no bearing on the need for medical advances. You can be of perfect health or a near death slob and still need penicillin to stave off infection. Either way you could die. Or how about appendicitis? People would die before we learned how to do the surgery. So you see HUMAN progress is growing in knowledge.

You could think of it as the process of evolution, but aplied to cultures instead of species.
