In general the y-intercept form is y = mx + b where m is the slope (the change in y divided by the change in x) and b is the y-intercept, where the line crosses the y axis and x is 0 (0,b)

The slope (m) of your equation y = (3/4)x + 5/2 is 3/4. Since it is a positive number the graph of the line will go up hill at a rate of 4 to right (horizontallly) and 3 up (vertically) it also will pass through the point (0,5/2) the y-intercept (b) where it crosses the y axis. ANSWER

Slope intercept form is always y=mx+b.
Therefore your example is, as has been said in slope intercept form. As was stated, 3/4 is the slope in this case. The slope determines the direction of the line as well as the angle.
Note that the value of b will always be the place where the line intersects the y axis when x=0.

If you were looking for standard or general form, which is ax +by=c, then you cannot have any fractions and 'a' must be positive.
