I don't know what the health risk is... but c'mon... 4 hours??!! I'm uncomfortable after 1/2 hour!! Even if a guy were able to satisfy a woman for 4 hours, and last that long... that's a lot of foreplay, and a lot of work!!! Even if he were to orgasm in a 30 minute time-frame... and then keep it up for another 3 1/2 hours??!! Good God no!! I'm getting wore out thinking about it!!

general rule of thumb (or shoudl I say penis) is that if an erection persists for four hours or more, you need to get ti checked by a doctor. And yes, there are complications (aside from the perpetual bulge in your pants) that can arise if the condition is left untreated.

Mainly that it is not going to go away by itself. The treatment for such a condition, called priapism, is severe. And the erection can be quite painful. The treatment isn't much fun, either. Check out http://www.webmd.com/erectile-dysfunction/erectile-dysfunction-priapism

is it caused by drug intake or it just happens?
have it checked! some leukemias present as prolonged erections....leukemic cells infiltrate the vessels of the penis, blocking venous flow causing prolonged erection.
