It would appear the this was done using enhancing filters and possibly the "Smug" tool. There was also allot of painting done to enhance the skin tones, and I think the eyes have been sharpened.

My advice would be to open the file, create a copy via Ctrl+J, and then just go to work. You can do the clean up work on the first copy and then create as many layers as is needed for the painting. Take your time, this kind of work doesn't happen with a couple of mouse clicks.

looks like they used airbrushing tool.
