In fact this does happen sometimes, but of course the affected people tend to hide the problem so the general public never here about it. Babies do get born sometimes where their gender cannot be determined, and there have been cases where the gender registered on their birth certificate turns out to be different from the way the person ends up.
It's a difficult subject but you should be able to find more info on the Internet.

It is rare. It is called a hermaphrodite. Most hermaphrodites will "choose" to be one or the other (rather mostly, the parents choose for them at birth) to help being in society. It is a rare genetic thing that happens when a baby is developing. I hope your friend can cope, it is quite a shock. That said, it changes nothing about his girlfriend's personality or the fact he liked to be around her, so hopefully he's mature enough to understand she was born this way and it doesn't make her less human or want to be loved any less.

Very unusual but can be entirely natural (or thanks to modern medical practices achievable by surgery). The term is hermaphrodite and can be caused by hormonal problems as the foetus is developing.
