It means, follow my train of thought, or understand the direction my thoughts are leading you. Usually expressed as a conditional, as in "if you catch my drift..." to see if you're keeping up with the tour group (if you catch my drift.) It means do you understand what I mean, where I am coming from, what I am saying, my attitude.

I don't think you're supposed to take it literally. It's another phrase for "Understand?"
