Heat bulb--a regular lightbulb in an aluminum dome reflector works fine. Attach it to a dimmer switch or rheostat to adjust to proper temperatures.
A UVB reptile bulb. Reptiglo or Reptisun are both good brands.
No heat mats needed--they warm from radiant heat, not belly heat like leopard geckos.
NO loose substrate or sand of any kind!
Branches/rocks safe to climb on and under.
Small water dish

It really depends on what size enclosure you have. You may want to read the following article for more info. There's an estimated wattage for different aquarium sizes that may help you. You may want to invest in a digital thermometer with a probe.
