A nuclear war would devastate ever inch of the world and could possibly wipe out the whole population. Those who weren't near the explosions would still have radiation poisoning that stays for hundreds of years. The survivors would probably only be cockroaches, and if humans made it, they would get cancer from the radiation and their children would be born deformed that they'd wish they hadn't survived.
This is why we don't want to risk Iran getting hold of some because their president is a psychopath that wants Isreal wiped off the map and hates the west.

There are never winners in any war.
A worldwide nuclear war would be a holocaust like mankind has never known before; those who died instantly would be the lucky ones. The rest would die slow, painful, agonizing deaths as a result of nuclear fallout, radiation, nuclear winter, and starvation.

None of them. Every major nuke containing country has another nuke containing country capable and ready to take them out. Once one country launches a missile, the targeted country will like launch a counterattack as soon as the first one is detected. Not only will the attacked country counter, but it's allies will most likely man their stations and launch as well. Once this happens, the original attacker will counter to those allies and alert its own allies to attacker. So there will be a bunch of countries launching nukes. In the end, everyone will be badly damaged, not to mention the radiation and in theory, all the top governmental and military bases will be wiped out as those will be the main targets.

So virtually every major country will be without a leader and therefore everyone loses.

One in a neighboring stellar system. Because of meteorlogical patterns (that is, providing they do not change), probably the least affected country would be Australia but with the rest of the world in chaos, they would lose too.

"Ant-land",.... or "cockroach-land"..... Most probably the only survivors that'll be left, if something horrible enough, like a worldwide nuclear war should ever happen.

After we dropped nukes on Japan a reporter asked Albert Einstein what kind of weapon would be used in WW3. He said "I'm not sure but, I do know what will be used in WW4...Sticks and stones."
