I need to get a copy of my medical records from a doctor whom i can't seem to find. office closed called 411.

i havent been to the office in maybe 4 yrs. i called the number but it's not them. i called 411 nothing. i've googled her name , yahoo, askjeeves. nothing. the old office info keeps coming up. i really need those records asap. the hospital she was affiliated with doesn't know where she is and they dont have my info cuz she operated out of a clinic. Pls help. i need those papers for insurance purposes.

You could try the Medical Association or whichever regulator which gives licences to medical practitioners. They should have her on their list. Unless she's retired or has been struck off.
