This happens all the time. I tell them I've been in a coma for 12 years and although the face is familiar, I can't quite recall the name or the position.
I just don't bother trying to remember names. It's so not worth it.

Friend of the old days and name forgotten. That's not applicable to me...but for those who may forget, per chance, ask straight, rather than beating round the bush, pleading that your memory has unfortunately deteriorated. Nothing unsocial there!

I'll say something like.....
"Well hey there ole' girl (or ole' boy) -- where in the world did you run off to?"
Then I'll just go on with the conversation and hope I drum up their name at some point. And if not, then oh well. I don't recall them calling me by name either!!

I don't think we HAVE to remember everyones name? Its no sin to be human! We have so many things we have to remember! Let me assure you this happens to me frequently. I try but if I just cant remember, I own up to it (and apologize!)

...I just come right out and say..."I'm so sorry, I should remember your name, but I don't...could you please tell me your name again?"...this sometimes gets a "look", but, oh well, it's better than making the situation more uncomfortable by faking it.
