No it's not really bad. A lot of people write "Application Essay". If you don't get in it won't be because you forgot a title. Messing up bad would be something like forgetting to write down your extracurricular activities, forgetting to send a transcript or lying on the application. They see so many essays that they probably won't notice a thing like that.

I don't think I've ever seen an application essay with a title, so if they didn't specifically ask for one, you should be fine. Even if they did ask for one, it doesn't seem like the most important thing in the world, so if you don't get accepted because you forgot a title (but everything else was fine), you didn't want to go to that school anyway.

I doubt that it's really an issue unless they specifically said that the essay must have a title. If you're really worried, give the admissions office a call/email and ask if you can submit a corrected copy of the essay.

I wouldn't imagine they are looking to see if you remember to put titles on your essays. They are probably more concerned with the quality of your writing, but it all depends on who is examining it. I would not worry too much about forgetting a title, though.
