Yes I did. About 20 years ago. A very inspiring story. I worked as a waitress 3rd shift at a truck stop, and all night long the drivers kept coming in talking about how bad the snow storm and roads were. So, when I went to leave the next morning, two drivers had left shortly before me. As I started driving, I thought, "This ain't so bad! What are they talking about!?". Then, I caught up to the two drivers that had left before me. I went to slow down and started fish tailing. Got it under control but was still going to fast catching up to them. I had to pass. The only problem was, it was right at an overpass where I couldn't see if any cars were coming. It was that or go under the rear end of the trailer. So, I started passing and lost control again. This time, I was heading straight under the middle of the semi trailer. Then, my car just stopped dead in it's tracks and the engine was OFF! I mean I was two inches from going under the trailer! Now, if that don't tell ya there is a God, I don't know what would. That is my one and only experience of spinning out.

Yes, it's simply going too fast for the amount of adhesion the tyres are providing.
Not knowing the road is icy causes this problem mainly unless you are a mad little devil or simply unskilled.
Also braking in a corner or lifting off the accelerator or pressing down to hard on it will also cause trouble. Just 'feather' it enough so the engine is neither driven by the wheels nor drives them.

Yes, twice, once I applied the brakes and one front wheel grabbed and I was 180dregs around. And recently I was accelerating on wet pavement. Both rear wheels lost traction and I was facing the cars to side of me on a 8 lane freeway.

About a dozen times a year, but I go a bit past "nearly" sometimes.

Closed course and professional driver naturally.

I have done better than that, I did spin out more than once on ice, luckily I never hit anything.
