Just tell them but tell them in the nicest possible way. Or offer to take them shopping.

Poor kid. Is this someone you know well? If not you need to be extra careful about embarrassing them. Instead of telling him/her to change the outfit, how about offering to take them shopping. If his/her parents don't care, there is the possibility that this is one of their only outfits(literally). Some parents don't bother to teach the importance of hygiene, and this person may not even realize how bad they are smelling. I can't figure out why the school hasn't talked to the parents. I work at a school and I know someone would have called the parents by now. Just to see if there was something that they could do. Something is going on with that child. I hope you can help without hurting his/her feelings. I would make sure you do whatever you decide privately, not in front of everyone else. Think of how you would feel if you were in this situation, and how you would want to be told.

I went to school with a guy like that. In truth, I thought he just had several versions of the same outfit, until the day his parents bought him new ones, and I realized how icky and dirty the other set really was.
Turns out, he was the son of the school janitor (I don't think there was a mom around)...His family didn't have the money to buy more clothes, or the means to do much in the way of laundry (he probably washed his once a week, but that was it).
It's entirely possible that this person hasn't had the opportunity to change or wash. Invite them over, and offer them the chance to have a good, hot shower while you put their clothes in the laundry. I'm certain it would be appreciated.

There is a guy where I work...we started the same day...he wore the same clothes everyday for about 7 months. He finally changed one day, and now hes been wearing these clothes ever since. Its a warehouse, so its not like he has to wear nice stuff, but still.

he or she is probably really poor and maybe cant afford clothes and they cant do anything about it just leave him or her alone you don't have to be there best friend! And tell your mom to talk to his and as if everything is alright but DO NOT EVER TELL HIM THAT WILL EMBARRASS HIM SOOOOOO BAD AND YOU WILL BE KNOWN AS THE MEAN WITCH!!!!

1st of all, how do you know this person hasn't washed their clothes?
2nd of all, you don't know their story. something tragic could've happened to him/her and they lost a lot of their clothes and possessions
maybe this person is in a depression and doesn't care what they look like
if this person is the same sex/size as you maybe you could befrend them and offer to share or donate some of your clothes to them

Maybe that person's house burned down in a fire.
Don't say something to them. It's not your business. Unless you are planning to help them out by buying them new clothing.
