Basic (and please note the punctuation!)
author last name, author first name. Title (underlined or italicized if a book / quote marks around it if shorter than a book). Publisher City: Publisher, Copyright year.

It would help if you made your question more specific, but basic MLA format for a book goes like this:
Author's last name, Author's first name or initials. Book Title [italics or underlined--no provision for that on this screen]. City of publication: Publishing company, year of publication.
Give the state (or country) as well as the city ONLY if the city is not well known or could be confused with another city of the same name.
For a periodical article it's like this:
Author's last name, Author's first name or initials. "Title of Article." Title of Periodical [italics or underlined]. Volume number if known [number only, Arabic numerals, not Roman].issue number if any (date; day if given month abbreviated year): pages.
Ex.:
Doe, John P. "Crisis in a Far-off Land." One World
3.4 (Apr. 2005): 11-18.
In the above example, every line except the first should be indented, but when I previewed what I just wrote, it didn't show up that way.
These are just the very basic instances, but any college English handbook should have a section on the subject. The Bible in the field is the MLA Handbook. These sites should also be helpful:
