It was mentioned when she met her birth mother played by Francis Fisher. She told Kerry that she was born with a birth defect.
This happened in season 11 in the episode titled "Just As I Am".
Later on, Kerry had surgery to her hip which corrected the problem and she no longer needed the crutch.
This happened in season 12 in the episode titled "Out on a Limb".

She has had to use a crutch ever since she joined the show. I believe that they said she was born with a bad hip. In a recent episode, she spoke of having a hip replacement, but she was having second thoughts because she saw her "limp and crutch" as kind of her trademark. She did not think she would be the same old Dr. Weaver with out needing to rely on a crutch. She had recently injured her hip, it apparently was not getting any better, causing her a lo of pain and discomfort, so she finally agreed to have the hip replacement surgery. Another reason she did not want to have surgery, is the fact that she will not be able to play, or even pick up her baby as she recuperates.

Yes it was a birth defect and not from any accident as many people thought previously. In the last ep. she had undergone a hip replacement. And hopefully will no longer need the crutch. And as bitchy as she can be, Dr. Weaver is one of my favourite ER docs.

Because she was Born with a badly displaced hip but she got it soughed out before she left last season!

I believe Kerry had hip dysplasia, which usually occurs in early childhood and can be corrected these days through surgery and splinting.
