Personally, I have not.
However I know of people who have had metal plates implanted for the purpose of 'nutting' people.
The steel plate really does increase the effectiveness of a well placed 'headbutt' and can reduce the risk of injury to the headbutter as well as cause more damage to the headbuttee.
I am also aware that many premiership footballers (soccer in US) have had this surgery for the purposes of heading the ball. -for increased range and protection from brain damage.
