Only three men won the VC twice (which means the same as saying “VC and Bar”).

[1] Captain Noel Godfrey Chavasse. Won VC twice during WW1. See link [1] below.

[2] Lt. Colonel Arthur Martin-Leake. Won VC in South African War (Boer War). Then again in WW1. See link [2] below.

[3] Captain Charles Hazlitt Upham. Won VC twice in WW2. See link [3] below.

Dunno, but the most common name of V.C. holders is Campbell, as a matter of interest.
