It has to do with the changes in the orbit of a satellite (say) due to the oblateness (i.e., not being a perfect sphere) of the earth. This is actually a pretty technical subject, so I point you here to get started:

Hi. Charles will get you started. A perfect, homogeneous sphere orbiting another perfect, homogeneous sphere would not have a J2 perturbation (assuming no interaction with a third or more body or force.) As the sphere deviates from perfect, or the density changes from place to place, an object in orbit will begin to have that orbit sped up in some locations and slowed down in others. This is the J2 perturbation.
