The best combo is a synthetic (Patagonia are the best) long underwear over cotton underwear. Boxers are OK, but those boxer-briefs are a little more comfortable. Different types of long underwear (light to expedition weight) can adjust the amount of warmth you need.

You can wear whatever you are most comfortable in. I generally just wear long johns, but I have also gone with boxers and sweatpants under my snowboard pants.
