To issue one, each state varies but most are 2 to 3 years.

Once issued, there is no time limit.... they never expire until the case is resolved... meaning the person is arrested.

Well, I can't answer as to every country in the world, but in the US, the UK, and pretty much any other country whose laws are based on English common law, the answer is "No" - a bench warrant, once issued, is valid till the subject is arrested.
