Zero, because I have more respect for people who talk about things realistically instead of making faces at everyone. Unless, and out of all fairness, they are cannot speak due to physical impairment at which time I luck out for knowing at least some sign language learned during my childhood. I guess if one cannot speak - no relation to refusing to speak - then I'd say that person is seriously concerned about something and would listen to their sign language as they spoke with their hands.
