If all of the accounts are locked out you may need to reinstall windows. This should preserve the other data on your hard drive. Place the windows disk into the drive and restart the computer. You may need to go into the BIOS settings by hitting one of the function keys while it is doing its initial system check. The exact key is different depending on your motherboard but it should tell you what it is as it is checking your memory and so forth. You will need to set the computer to load from the CD drive before the hard drive.

Start ur PC> Press F8 couple of times while booting . then u will be provided a list of menus Select Safe Mode. U will se there is an extra user name called Administrator log in as Administrator ( if its password is not changed, normally it can not be changed in normal accounts), then go to the user accounts option and change all the passwords. And one thing create a Limited account for your 5year old so she cant change the password anymore .

Never leave a child unattended on a computer, especially one as young as 5. My son is 13 and I still check on him every 5 minutes (and our computer is in the family room). I also don't believe for a moment that the 5 year old did it. You better ask your 14 year old son again.

In a typical Windows XP machine, even with NTFS, it does not start up securely unless you tweak it, settings and files of one account can easily be copied to another, so what I would do is installing an extra install of XP alongside this messed up one and copy things.
But that's just my way as a fallback, you should try the options of bootup F8.

Ask her what she changed them to.If she won't tell you,ground her or take away something that she really likes until she does.Anyway,she had to know your password in order to change it so don't let her know what is is.
What's the point in having a password if it's not a secret?
