Hannibal is the most obvious, but I like Mad Dog Cole from the movie Mobsters. He bit off a nose, cut out a tongue, stuck an icepick into somebody's brain and went around shooting people with a Tommy Gun in each hand. The best part was that he laughed the whole time.
