The Philippines was once a rich and developing country.. it was connoted this way because the country is now, i may say, struggling for economic stability.. being The Sick Man of Asia is related more on the economy of the Philippines... and it does not only go for as a connotation for China in the 19th century. as a matter of fact, Afghanistan was also called this way. i believe it is used to call a country that is in crisis, particularly as an economic-related connotation.
by the way, Pearl of the Orient is the real & original "quote" name for the Philippines, it is not Hong Kong's...

The phrase "sick man of asia" refers to the Chinese empire during the late 19th century and early 20th century. It is not a reference to the Philippines.

Well, the Philippines got its name from the Kings of Spain which colonized it in the 16th Century. I visited Manila nine years ago and my lasting memory is the number of passport photographers who advertised the availability of sailor's suits for purposes of fraudulent emigration. I wasn't aware of the "sick man of Asia" sobriquet at the time, but it doesn't surprise me now.
