More to the point, why don't you believe her? Is she a compulsive liar or something?
But to answer your Q: You won't necessariy be able to tell as an observer as not all women get 'symptoms' i.e vomiting. And most pregnancies won't start to show until around 5 months, but again, this varies from woman to woman. I'm more interested as to why you dont believe your sister?

I'm not sure why your sister would lie about being pregnant, but ask to see the pregnancy test confirming it. She needs to go to the doctor if she is pregnant so they can confirm it and make sure her and the baby are healthy. She won't start showing for a while, but she may have symptoms (like morning sickness).

Does she have a positive pregnancy test? Other signs to look for can include morning sickness, sore chest, mood swings, headaches, and constipation. I hope she wouldnt lie about being pregnant.

Ask her to take a pregnancy test, if she is sick for a while, no feminine products in the trash. Or you could just trush her, why would she make up something like that.

Is she having morning sickness, r u having the sickness for her? Is her completion lighter or brighter? Is she eating abnormal? Is she quieter? She show u her test? Is ur mom sick every morning and knows its not a flu or cold but feels strange?

If she's trustworthy then there you go. If she's not, then give her a tester and tell her you want to see how the color changes so she's not suspicious.
