In English, yes. In arabic, though, I don't think it ever is (Iraq, Qatar, Qom, etc.)

Usually, but not always. I am reading a book with the main character named Qwilleran. I believe there is also one or two others also. Qatar (a country in middle east) and Qiana (a nylon fiber).

most of the time in the English Language. There are one or two exeptions, which are forgein words.
