Naruto's father is Namikaze Minato, also known as the Fourth Hokage (Yondaime Hokage) and Konoha's Yellow Flash (Konoha no Kiiroi Senko). So yes, the Fourth is Naruto's father.
There are speculations about the second question. But I believe he is dead, as a result of sealing Kyuubi no Yoko. Some popular beliefs are "he's Akatsuki Leader", "he's currently in Death God's stomach but will return when the time comes", and "he's still alive, just that he doesn't want to show up yet".
Naruto's mother's name is Uzumaki Kushina, a kunoichi from former Whirlpool Country (Uzu no Kuni).

okei to clear up some rumors and everythin here's the whole explanation to the followin:
1) Yondaime Hokage, student of Jiraiya and Konoha's Yellow Flash, is the father of Naruto. There are speculations that Naruto's dad is Kazama Arashi but actually its just a fake and the rumor of Yondaime bein Pein? Its not true cuz the "supposed" manga chapter that was shown was a complete fake...just edited it with old naruto pictures.....but anyway....Naruto's dad is Namikaze Minato or the 4th himself. He got his father's looks(obviously) and his father's determination to become successfull and to protect those dear to him. His mother on the other hand was none othe than Uzumaki Kushina. Uzumaki Kushina was a formerl Whirlpool nin and her tomboyish behavior and ninjutsu type was passed down to Naruto. Both of Naruto's parents are dead because Naruto won't be an orphan if he had his mother. The leader of Akatsuki, Pain/Pein, is Nagato and his hair style is similar to Minato's but I think the body that Pain is usin is the body of Yahiko the real look a like of Minato only with diff. hair color...thats all...

The Fourth Hokage is Naruto's dad. This has been revealed fairly recently in the manga. At the same time, they give the name of his mother, and mention that she was a tomboy with the same fiery personality as Naruto, and was from the Whirlpool Village. She apparently died at some point before Naruto was old enough to really get to know her.
As for the question of whether the Fourth is still alive, officially he is dead. But you never know what plot twist might get written to reveal that this is not the case.

i can answer the part which is if naruto's dad is still alive...
*spoiler ahead (this came from one of the manga that was released)
naruto's dad (the fourth hokage) is STILL ALIVE he was later revealed the leader of the group akatsuki (or member, i dont quite remember that part) he showed himself to naruto which was at that time with jiraya... naruto can't believe it at first.. and then he got mad at his dad for being one of the akatsuki and much worse the leader... there he first saw his dad and met... jiraya was not that surprise since from the start he knows that the fourth hokage is naruto's dad.. but he doesn't know he was still alive since he used a technique which will cause your life to the nine-tails fox do you want to know why naruto's dad became an akatsuki... or build that group?

since the day naruto's dad fought against the nine-tails and was forced to put it to naruto who was just born at that time he hold a grudge against konoha... since he knows if he put it to naruto (his son) it will affect naruto's life (which really did happen)... so he hated konoha and he purposely let the village think he was already dead.. and there was also a part in the manga that he also said konoha was the reason why his wife (naruto's mom) died.. the reason? i dont know^_^

the 4th hokage (Naruto's dad) is dead. at least he supossed to be dead already (because of the side effect of sealing kyubi).
naruto's mom, we don't know yet where is she. But I think she's still alive.
What we know about his mom is, her fighting style is like naruto's.
so maybe, she uses also "naruto rendan".. well maybe "naruto no oka-san rendan" LOL
