pseudonym comes to mind and alias, Nom de plume, nom de guerre, A.K.A., pen name ,stage name or fake name, née, ne, moniker. Which of these is the one you forget.
Then again maybe you are thinking of a diminutive of a name . Bob for Robert
or Trish for Patricia , Diminutive is that word for that kind of self given nickname.
