What is the motive for companies to use a Separation and Release Form when firing an employee?

Because a) It makes sure that the person doing the seperation doesn't miss something that they need to do, and b) By *only* making space for the things they need to do, it prevents some middle manager waffling on in a free-form letter and writing things that could be used against the company later.

For example, if the form says "Reason for termination?" and has check boxes for half a dozen - all legal - reasons for the manager to pick the right one, you don;t get a manager writing a letter that says "You're really too old to fit in here" and getting the firm sued for age discrimination.
