I went to staples and purchased a receipt book. But I don't know how to fill it out. This is my first time. It looks like a cheque sort of. It says FOR..is that the employer's name? And then at the bottom it says FROM..and TO. Would that be my employers name again? And then it asks for TAX REG. NO. What is that?? And then next to it it says BY. I'm so confused. And I'm working tomorrow and she'll ask me to give her a receipt. Is it okay if I leave the tax reg. no. blank?
I hope someone can help me.

It depends on why you need to provide a receipt. If what ever is being purchased is taxable, you need to find out the tax percentage, add it to what ever is being charged, and give a total for payment, and when you get paid, you mark it as paid. You will have to report the earnings, and taxes when you file your taxes. If you do not understand, then you need to consult a tax specialist to insure that you are not making any mistakes, and you will not end up penalized later by the IRS.

If you are working in her home, by law you are most likely a household employee, not an independent contractor - sounds like she's trying to have it both ways, evade employer taxes and stick you with the extra social security tax that she should be paying, and claim the payments for child care on her taxes.

That said, to fill out the receipt, you'd put the amount, "for" would be babysitting, "from" would be your employer's name, and "to" would be your name. She will be looking for your social security number in the tax number area - she'd need that to claim the payment on her tax return.

You don't really need a receipt book - just typing something up that says something like

"Date xx/xx/xxxx
Amount xxxxxxx
Received from (her name)
for babysitting
by (your name).
