my mother had a friend whose birthday was on the 29th of February... I think the birthday was only celebrated every Feb 29th, actually.

I was 10 years old when my mom was 40, and she thought it quite funny to bewilder me by saying her friend was turning 10 that year. As I didn't know her friend, all I could think of, as a person who could possibly be her friend, was one of my classmates LOL It hit me a few minutes later as to what she really meant by her friend turning "10" though!

Though typically I think the birthday would be celebrated either the day before, or the day after... whatever the person's family decides on.

Every year he'll get one year older, no matter the date. So, as someone has already said, either on Feb, 28 or on Mar, 1

I would personally celebrate it on Feb. 28th so it's still in the same month.
