Mine is while I'm on long trips, falling asleep at the wheel and crashing over a bridge, knocking myself unconscious and drowning.

The water thing you mentioned is a close second though but my fear is being trapped AWAKE in a submerged car. I have been begging my husband to buy one of those "window breaking/seat belt cutting" tools that I saw on myth busters. They actually work. I would keep it in my center console (which has a cover) so it would be handy if I needed it.

I have a fear of not being able to hang on to my children during a tornado. Tornadoes and spiders are reoccurring nightmares for me.
