STOP beating yourself up and telling yourself that you're lazy. You just need to get into the habit of studying regularly. Start with 15 minutes for each subject and slowly move it up. Start with the subject you're most interested in or you find easiest. Tell yourself that you can do it. Most importantly is to think positively and do not give up on yourself.
Do you have a friend that you can study with? Is your room/house too full of other distractions? If so go somewhere else quiet. Maybe you're not cut out to for the books but everyone has a unique gift and you must find yours and make a full life out of it. Go find yours. Do it now.

The best way to break the habit of laziness is simply to get into a habit of doing.
Set an alarm clock for your study time. Set up an area on your desk with the books or tools, etc, neat and available; a dedicated work area is one of the most important gifts you can give yourself. Exclude distractions such as the television, unrelated web surfing, family, suchlike. When the alarm goes off, GO THERE, no exceptions.
Allot yourself ten or fifteen minutes per subject; use an egg timer. Even if you break concentration and don't study for all that time, or don't even crack the book, obligate yourself to sit there for the full time.
It may take a while, but you will eventually teach yourself that that time is study time, and it'll be easier for you to get it done.

Dont listen to the negative comments. This is actually very common is alot of people. Finding motivation can be really hard. Everyone is different. Procrastination has actually been found to be a mental health thing. Nothing serious of course but with some therapy it can be dealt with well. Thats just an idea. Other than that, i personally find sudying in a clean, quiet environment to be pretty good. Set your self some goals (realistic ones) and reward your self, whether that be going out with friends. ANYTHING. Be creative with study methods. Plain reading from text books can sometimes be way too boring and dragging.

Stop making up excuses and think about the consequences of your actions. What will happen if you fail your studies? What kind of future will you have if you don't apply yourself now? Do you want to be flipping burgers at some burger joint for the rest of your life, or perhaps you'd rather be in some parking lot going through the garbage cans looking for empty returnables. Sounds like a plan to me!

But that would be soooooo much work.
The short answer is that laziness doesn't really exist. Everybody does what they think is fun, and avoids what they think is painful.
You no doubt could play/study a video game for hours to perfect your characters skill.
Got it? You interpret 'Math' as pain, so you try and avoid it.
If you saw it as helpful, useful, fun or a neat puzzle, you would study it.
