A phase change diagram shows what phase of matter (solid, liquid, gas) a substance has at given conditions of temperature and pressure. Phase change is a physical change, not chemical. A phase diagram can also show changes in phases and their makeups in an alloy (or solid solution) of two metals (or a metal and a nonmetal).
