it has been kept in my basement, which is climate controlled...not hot or cold...

it was not shaken. it was like that as soon as i opened the can...it doesn't stir well, the settlement on the bottom just attaches to the stick...about 2 inches of goo on the bottom. im gonna buy a new can, i was just curious about it....

It was subjected to extreme heat or cold. I would buy a new can. I've kept polyurethane in the garage and we get triple digit temps in the summer. I didn't realize (but it says on the can) not to subject the contents to extreme temperatures or it will affect performance. It did because I went ahead and used it with "cloudy" results that were unacceptable. Big pain to remove it and start again...

Did you shake the can before opening it? Urethane should be stirred, not shaken. I don't know if this is the case, but just a thought. If you did shake it, try letting it sit for a while.

Sounds like it drew moisture. I would not use it as you may need to remove and resurface the item you used it on. You could always stir it up and try it on a test piece of scrap wood to see how it reacts.

If this is water based poly it will be "milky" or "cloudy" it will go on that way and then dry fine, try it before you dump it.
