4. True, although I prefer "polyhedra" as the plural of "polyhedron."

5. False. This describes a cylinder.

14. False, because the description is insufficient. A slant height must go from the vertex to the midpoint of a side of the base.

15. True. In fact, this definition is overly restrictive, because any rectangular prism or parallelepiped is a hexahedron, and so is a pentagonal pyramid. A cube can also be called a regular hexahedron.
