The answer is different for every woman. To me it is not one thing. I always look into his eyes, and his smile is of the most importance to me. I also have a tendency for checking out his arms and hands. Strong hands and arms are very sexy to me. That would be a nice beginning, however I can talk about beauty of a male body for hours. I pay attention not so much to what he wears, but how he wears it. I can guess how self confident he is simply by watching him walk toward me. BTW, I don't find men who wear accessories ( other than a watch, tie and a belt ) appealing.

Stereotypes don't come from no where...surely you have seen a woman with a new outfit and the man doesn't even notice. Believe me if you have a guy noticing these little details I suspect you will be competing with him for face time in front of the mirror. I read a book once that suggested that on a first date guys should worry a lot more about their clothes choice and women should worry a lot less. If your clothes are flattering to your complexion, hair, and figure you've covered your bases. All of the other stuff is for other women.

In order: Eyes, Mouth, Arms and Chest. Then I work my way down from there.
Love a good bear hug, so I like a strong pair of arms (but not TOO muscular... I like to hold a MAN not a brick building!)
His clothes don't really matter too much. As long as he's clean, I'm pretty happy.
Not really much into a man wearing too many accessories... just a watch, one ring and one chain or something. And no piercings! That's a turn off for me.
Don't much like body art either, but maybe one tattoo's okay.

Eyes first. Then hands. I hate soft hands on a man. Legs. Don't really pay attention to accessories, unless he has an earring--love that. I don't mind tattoos either.

I know that I personally notice a man's teeth/smile, hair, and eyes most. And I also notice how they dress. Not really one piece of clothing, but just how well put together they look. But I know that if a guy has messed up teeth, it's a turn off to me. I have dated a guy in the past with messed up teeth, so I can get past it, but they defiantly make a 1st impression for me.

His hair, eyes, lips, chest, arms, stomach, and legs.
Shirt, jeans, and leather pants.
If he has firm, solid, lean rock hard muscle tone he is in excellent physical shape.
I also notice his teeth, and hygenie.
If he has gross teeth then obviously he isn't very sanitary, caring, and considerate.

I notice men's hands. Hands show what type of work you do. how meticulous you are about your grooming (manicure), and if you are married.
I also notice shoes. Shoes are sometimes a forgotten detail. The type of shoe, the condition of the shoes tell you a lot about people, for the same reasons.
