This slab (without a perimter footing) would take 2.25 yards of concrete. Concrete in NE Texas costs about $100 per yard plus tax. In addition, since it would be considered a small pour, there would be an additional charge for the under-sized load.

4 yards is double what you need! Heres the math:
12'=144 inches, so 144x144x5=103,680 cubic inches.
divide that by 1,728 cubic inches per cubic foot.
that equals 60 cubic feet, divide that by 27 cubic feet per cubic yard, equals 2.22 yards of concrete. that's in a perfect world, order 2 1/2 yards. as for price, here in the Midwest its $80 a yard. After the 1st of Nov. they charge $4 more per yard for hot water, and if you want it to cure fast you can add calcium which for 1% calcium is another $2 per yard or 2% is an extra $4 per yard. Maybe a little more info than you need, but hey I'm in the buisness. I've poured out hundreds of thousands of tons of concrete!

What I would do is call a ready mix plant and just ask them how much it would take and cost.

You could have all the answers you need in a 45 second phone call.

mirhc is right.

Divide the sqft by 64.8 for 5" slab

81 for 4" slab, 54 for 6" slab

The math is 144 sq.ft / 64.8 is 2.222 yards.

The price per yard will vary, but 65-75 / yard is reasonable.
