Does this happen because of corporate incompetence, or someone's idea of a joke? Most of the people on the list seem to feel it is because the people who are delivering the flawed services really don't care.
A lot of people are comfortable with how things now are, or were, and don't want change to something else, so they sabotage the change. This is so prevalent, that part of the standard management of any change is to deal with such people.
There can be rage in the work place, frustration. How do you deal with it to keep sane? The worst cases go postal ... get a gun to work place and shoot a bunch of people. We are seeing this more in schools nowadays for complex reasons partly due to US government rule changes to save tax money.
Remember when drugs were easy to open the containers, before they had to be made tamper-proof? Remember the Tylenoil reason why that had to be done? Some nut goes into drug store, surrepticiously messes with the containers of drugs on the shelf not sold yet, then whoever buys that is poisoned.
