A couple changes of clothes
Extra socks (they get lost)
Burp pads
If breastfeeding, a cover cloth
receiving blanket
enough diapers (figure about one per hour then add two)
Wipes
Pacifier
Extra thermometer
Any meds baby is on
Extra hat
I also throw in a few items for mommy too:
Pads (if you're still bleeding)
Tylenol for adults
I hope I havent' forgotten anything.

Assuming baby's diaper gets changed every 2-3 hours, put enough for the length of time you'll be gone, plus 1 or 2 extras, just in case. Baby wipes, 1 packet. Blankets, 2--one to cover up and a spare in case of leaking. Changing mat, 1 (unless disposable). I always carried two changes--warm and cool, if the weather was not certain. But if baby leaks quite often, then take at least one more. Always have 1 or 2 toys. Bags for the dirty diapers, 1 each. Breast pads, for you. Number depends on how much you are/expect leaking.

I've never really carried a diaper bag unless I was going somewhere on an all day trip.. I breastfed 3 of my 4 children. I do make sure to always have a change of clothes in my car for each of the kids. Otherwise, I usually just carry a travel pack of wipes, some extra diapers and hand sanitizer in my purse.

I would say at least two changes for a short outings, because newborns tend to leak through their diapers, especially breastfed babies. If you are going out for a night, then more. I would put several diapers, wipes, a changing pad. Maybe some baby powder, this is easier to put on then cream when you are out. Bring a bag to put dirty diapers in.

One or two additional outfits. If it's cool something warmer, if it's hot something cooler as options. Wipes, diapers. How many depends on the length of your outing. A quick trip to the store two or three, longer outings eight to ten. Powder, diaper rash cream. Pacifers if you use them. Extra bottles (if you're pumping). Breast pump if it's a long outing and you are pumping for bottles. A changing pad, burp clothes, receiving blanket.
Sounds like a lot, but you'll get the hang of it.

Here's what's in my diaper bag:
4 diapers
wipes (I put some from my big pack into a ziplock bag)
1 change of clothes
hat
socks
disposable changing pads
moist towelettes
mini first aid kit
disposable place mat*
take 'n toss spoon*
1 container of baby food* *(for older babies)
on the go trash bags
travel size sunscreen
travel size bug spray*
travel size lotion
diaper rash cream
bib
2 small toys

i always pack a receiving banket for breastfeeding in public i would use that to cover up. i would bring 2 outfits(because my son peed on everything) i would bring 1 diaper fpr each hour -hour and half i was gone. nipple cream breastpads, my diaper bag has never been too big since i breastfeed, hes almost 7 months

about 6 diapers,diaper wipes,and spare t-shirt that can be worn for mommy or daddy in case of a wetting accident,a spare onsie,a receiving blanket,a bib,a burpie cloth;I know you plan to breastfeed but if you are unable :

make sure its insulated (get it free at dr. office or hospital from enfamil insulated bag.)
i keep plenty diapers, cream, wipes, 3 outfits, 1 bib, pacifiers, poweder, milk bottles with ice pack (can only be in ice pack for 6 hours before going bad!!!be careful traveling with breast milk) other then that you might not need much more til hes older and needing toys and things...
