Well, my given name is Katherine Elizabeth, and when I was young my southern relatives called me Katie Beth. Now that I'm older, I go by Kate or Kaye, or just Katherine. She could use Kathy, Kat, or whatever she wanted if she decided she doesn't like Katie.
I would advise against using Katie as a given name. Doctor Katie Smith sounds very silly. Why not use a name like Katherine, Kathleen, Katarina, Katriel, Kate, or even Acadia for a given name and call her Katie? It will give her a choice when she grows up.
