Things like "you know", "like", "um" etc. are habitual verbal tics that are very difficult to eliminate. Many people aren't even aware that they are saying these types of things. Some people use these words as a filler to allow them to think, or to try to engage their conversation partner. I think people who say "you know what I'm saying" are making sure that you are following their part of the conversation. They look for reassurance you are with them, and if they don't see it, they ask the question. Mostly, however, it is an unconscious habit. If you point it out to someone, they may become more aware of it. If they care about it, they may want to change, and you could even let them know when they slip and let it out.

Verbal tics can be very annoying in others, but we should remember that no one is perfect, including ourselves!

John Maynard Keynes said :

"Words ought to be a little wild for they are the assault of thoughts on the unthinking"

the thinking process is evolving millions of years ago, unlike the language which is evolving only 10-15 thousands years.

for example the baby cant express because he doesnt have much words.

The same applies to us, we cant explain hard ideas because we dont have enough words in any language !, you know what I mean ! :P

They probably feel really uncomfortable and want constant reassurance that people won't give them weird looks and walk away when they're done talking. If they say "You know what I'm saying?" after every sentence the second someone says "No" they can be like "Just kidding, just kidding."

Its out of habit. People are really stupid, and it is difficult to understand some. Its the same way with routines and such. People do things because they can't get used to anything else.

Another reason could be that they don't want to finish their sentence or don't know what else to say.
