At its roots, the ISO 9000 document is a statement of how the organization does its thing. The object is to accurately outline what everyone does.

Enterprises are hierarcharchical.

People are to do things, take initiative, if the higher ups in the organization want stuff done.

Whether lower down people inform higher up people of some situation, really depends on the expressed needs of the higher ups, not on the situation.

This is expected for good communications. Quality assurance is expected in any business but especially in manufacturing. It is simply good management and supervision practice to keep the director apprised of the quality of the product. It is not needed in the IS09000 document but it doesn't make the document worthless as the document simply states the standards.
