I would go with James on this one, butterfly nuts with bolt, make a small hole where you want the paper holder , put the bolt thru the holder then screw on the butterfly nut and push nut thru the hole, and tighten the bolt, you may have to pull backwards on the paper hloder to get the butterfly nut to touch the inside of the wall before you screw in the bolt,,, you can do this it's easy

You need a hollow wall anchor. Otherwise known as a Butterfly or "Molly" style connectors. The Molly's expand as the screw is driven in, and thus keep hold better than a plain screw. The butterfly is for larger heavier pieces.
If you've tried the butterfly and molly before with poor results, consider the way in which they are installed. I typically use a drill for mounting a molly, with the drill size SMALLER than the size oif the molly's widest opening. In this way the molly must be pressed into the hole with a bit of force, assuring a better fit. Then when the screw is driven in, the molly expands furter, making for a very tight hold. Molly's are great for holidng such lightweight objects as paper towell holders - so long as they are installed well.

Get an anchor called EZ Anchor at any home improvement/hardware store. They cost a little more, but can be easily screwed right into the drywall, and they hold a LOT of weight on them. Used them to hold up 4 foot long wood CD shelves with metal brackets.

instead of an insert buy a butterfly that once pushe in expands over a wider area behind the wall, ask ur local hardware guy, stay out of wal-mart
