Do a little.
Pick one small thing to change and then habitually do it. For me it was "putting away my shoes." I would kick them off, then when I needed shoes I'd grab another pair. Sometimes by the end of the day there would be three pair scattered about the house. Of course, many things in my life were out of sorts. Including the LISTS others have suggested you create!
So I chose one small thing to change. I put away my shoes every day. I wouldn't go to bed if there were a pair of shoes out anywhere. Eventually, I could also put away my clothes! lol!
Success in any area of your life can be transferred to any other area of your life. For example, if you can clean out your closet, you can also lose weight. If you can clear off your desk, you can also shower and put on completely clean clothes--or vice versa.
The important thing to remember in trying to effect change in your life is not to bite off too much. Don't delay a project because it is too big. Break it into small pieces. For example, don't say, "Next week I will buy a new car." Instead say, "Next week I will begin researching what kind of car I want to buy."
This is one of the biggest barriers to getting things done: taking the "all or nothing" approach.
Here's another example. HOW TO CLEAN YOUR DESK IF YOU'RE A PROCRASTINATOR
1) Pick a date by which the desk will be clean, for example, Friday.
2) Today's Monday. Put all the dishes that are on the desk back into the kitchen. Reshelve books you've pulled out.
3) Tuesday, put all the pens and pencils back in their jar. Paperclips back in the cup. All the little "supplies" back where they go.
4) Wed. Stack the papers. If they are too hard to sort, put them all into one stack and pick a day next week to sort the top half inch of the stack. Otherwise, make an Urgent stack, Important stack, and Maybe later stack.
5) Thurs. Remove clothing, trash, gumwrappers, anything else that does not belong on your desk. If you don't have time to put it all away, put it in a box to deal with later. Yes, you're putting this off, but the current task is just to clean the desk.
6) Friday, dust and wipe down the desk so it looks really nice. Put some flowers on it, or something beautiful to make it an enjoyable place to work. You deserve the reward, and the beauty will help you keep it neat.
7) Next Monday: decide which day you will deal with the pile of papers or the box of junk. ;-)

Most individuals procrastinate most of the time. No it is not easy not to procrastinate because most things we need to do are boring and require some effort. You can limit the times you procrastinate if you join the military or be out of a job and have no place to live. When we have to do things that are life threatening we will not procrastinate believe me. So, try taking a job where if you don't move or get out of the way, you may get hurt this is a good way to cure what ails you.

I sorta have the same problem (like millions of other people), what has helped me is making a list and a schedule of when I am going to accomplish my tasks. Like for today, I took 10:30 a.m. to 11:30 a.m. for free time and during this time I get a chance to free my mind before I move onto my next task. I schedule to do a task this morning from 9-9:30 to focus on this task and if I didn't get it done, then I put it to the side and schedule to get this done in the afternoon or during my free time. Also learn to prioritize what tasks need to be worked on first and what can take time. I hope this helps and doesn't confuse you too much, but it works for me.

Make yourself at least start. If it's cleaning out your closet or working on a paper, you're more apt to working longer if you are already doing something with it! Get yourself to get up and begin the work; chances are, you'll get further than you would have expected AND you won't be kicking youself for taking so long to get the job done!
