In the wild they are worn down on their own.
In captivity they tend to do the same thing depending on the type of surface the animal is given to live/walk on. However if push comes to shove and they need taken care of by a keeper they are not clipped, they are just filed down.
Keepers have to keep a close eye on the feet of elephants because it can be the stem of many health problems for the animal. To boot elephants are one of the few animals that cannot walk on three legs so if something was to happen to one of the feet the animal would need to be put down.
For the most part though it happens on its own.

well u need not clip its toe nails coz they never grow tat long u can clip them.
so dont get troubled too much or u might crushed.

I imagine it would not be very different from trimming the hooves on horses, using a trimming knfe and rasp.
