Fred Haise was the astronaut shown throwing up in the movie. I don't know if he really did, but apparently, a lot of astronauts do. It's due to motion sickness caused by weightlessness. This is caused when our inner ears cannot figure out which way is "UP" and which way is "DOWN". If you close your eyes and tilt your head forward or sideways, even with your eyes closed you can tell that your head is not positioned upright. This is because your inner ears can sense which way gravity is pulling. People at sea on a stormy ocean and astronauts in space in weightlessness (extreme microgravity actually) cannot rely on their inner ears to tell them this and their brains get scrambled signals that starts the motion sickness.

Many astronauts get space sick and throw up. It is a form of motion sickness, like air sickness or sea sickness. I don't know which Apollo 13 astronaut got sick, but I know one is shown throwing up in the movie.

Space sickness (similar to motion sickness) is common, most astronauts experience some form of it in the first few days of a space mission.

Bill Paxton who played "Fred Haise" (one of the three astronauts on the movie Apollo 13) puked. I doubt they made it up just for the movie.
