Snip off the outside end with a pair of cutters and push the rest into the wall. Then spackle the wall hole.

Why are you removing them? It would be much easier to fill them and paint over them... If you are intent on taking them out, I have found that bending the outside edges up and grabbing them with a pair of needlenose pliers works okay.

Both 1 & 2 are valid.

If they are just the press in type; you could re-insert a screw and pull them. If they are the better/screw in type just use a screw driver and reverse the threading. In any case you'll have to patch the holes

Hammer them into the wall and fill over them is the easiest, unless they will pull out easily.
