One possible origin:
Signing up for chat rooms and forums. Say you want to create a yahoo ID named "newguy", but you find that it's already taken. You can either put a string of numbers after the name, such as newguy54934 (but that's generally frowned upon these days) or you can try different variations of upper-case and lower-case combinations (NewGuy, nEWgUY, etc.).
This is no longer possible in most online communities (such as Yahoo), as the names aren't case sensitive anymore.
Another possible origin:
Programmers tend to string words together when creating variable labels, capitalizing the first letter of each word except for the first one.(such as: variableLabelOne, textBoxName, etc.). Some programmers tend to type like that when they're chatting or posting in forums. Kids and "Noobs" might have seen that and had no idea WHY they were doing that, but thought that if they typed like that they would look like "hackerz", sO it eVEntuaLly MuTatED intO THis.

I know in my case it happens when I try to touch type very fast and my left pinky hits the CAPS LOCK key and shifts me to capitals or when my finger hits the second letter in a word before I released the Shift key. Spell check will always offer to fix that.
